Spy Eye Loss

UN rules question: if I use spy eye, and my opponent has no cards in their library, do I lose?

Yes. The rules simply state you lose if you draw from a library with no cards in it.

Drawing from Empty Libraries

If my opponent has an empty library and deal combat damage to them with Spy Eye, can I choose to draw a card from their library and thus lose by trying to draw a card from an empty library?

You lose whenever you attempt to draw but cannot. It doesn’t matter if it’s from your own library.

Hazmat Suit Rule

Me and my friends are finally drafting unstable, and I cast Hazmat Suit (Used) on his Spy Eye, and shenanigans resulted in his mom chasing his brother with the spy eye to make the brother touch it and take damage

There’s a rule that you lose life for touching Hazmat Suit not it touching you.
