If green is the color of destiny then why is Triad of Fates B/W?

Because most of Theros would be green if you let it (much as most of Innistrad would be black).

Was design aware that Oblivion Stone uses Fate counters when they made Triad of Fates or was this interaction an accident?

Very metaphysical question: Is Design aware of fate? I’m going to go with yes.

Triad of Fates is 3/3 and has three abilities, but has a CMC of 4. WHY MARO WHY?

It originally had a CMC of 3, but I assume it was too good. Even the Fates must bow to Development.
